  • Patients are bulk-billed where eligible.

What the Service Focuses on

Our Wellness & Weight Management service provides structured medical care for patients seeking support with:

  • Weight management
  • Metabolic health
  • Lifestyle related health optimisation

Metabolic health underpins many chronic conditions, including diabetes, cardiovascular disease, fatty liver disease, hormonal disorders, and insulin resistance. Because metabolic health plays such a central role in long term wellbeing, our approach focuses on addressing the underlying factors that contribute to weight gain and metabolic imbalance, rather than treating weight in isolation. Care within this service is holistic and supportive, combining medical assessment with practical guidance and ongoing support to help patients make sustainable changes over time.

As part of this program, we offer ongoing support, education and resources to help you make sustainable lifestyle changes. This may include practical resources, workshops, talks, and other initiatives designed to support patients throughout their weight loss journey.

Our focus on this area reflects our belief that addressing obesity and metabolic health can improve overall wellbeing, reduce future disease risk, and support long term health outcomes.

A Specialised Medical Service - Not General GP Primary Care

This is a specialised Nurse Practitioner led service with a defined clinical focus and is not a replacement for general primary care.  Patients may still require a GP or other healthcare providers (specialists or allied health) for unrelated medical concerns as referrals form a routine and appropriate part of care.

Access & Appointments

  • BULK-BILING: All in-person consultations are bulk-billed for Medicare eligible patients.
  • NEW PATIENTS: New patients are welcome and currently being accepted for this service.
  • TELE-HEALTH: Telehealth appointments are available for current and new patients. If you have not attended the Practice in-person in the last 12 months, a private fee of $50 will apply.

Who Can Access this Service

Adults may self-present for assessment and do not require a referral to access this service. All care is assessment-based, and suitability for treatment is determined following clinical review.

While we launch our Wellness & Weight Management service over the coming weeks, St Clair Medical Practice is operating limited clinical hours for consultations and care related to this specialised service.

Current clinical hours:

  • Monday: 9:00am – 2:00pm

How Care is Provided

Assessment led and Individualised

There is no standard or automatic treatment pathway. Care within this service is guided by individual clinical assessment.

Your clinician may consider:

  • Your medical history
  • Weight and metabolic health context
  • Current health conditions
  • Medications you are taking
  • Relevant test results
  • Safety and suitability

Attendance does not guarantee any particular treatment. All decisions are based on clinical appropriateness and individual circumstances.

Medication as Part of Care

As part of comprehensive weight-management care, medication may be considered where clinically appropriate following assessment.

This may include weight-loss injections such as GLP-1–based weight loss injections, where suitable and safe for the individual.

Medication:

  • Is a support tool, when clinically indicated
  • Does not replace lifestyle measures
  • Is not suitable for everyone
  • May be adjusted, paused, or stopped over time

Response varies between individuals, and progress may be gradual. Safety always guides treatment decisions. Importantly, weight management within this service is viewed as more than medication alone. Patients are supported with guidance around lifestyle, nutrition, and overall wellbeing as part of a comprehensive approach to care.

Ongoing Support & Wellbeing Focus

Our Wellness & Weight-Management service is designed to support patients beyond the initial consultation and medication.

As part of care, patients may receive:

  • Ongoing monitoring and support
  • Practical resources and education
  • Guidance around nutrition, movement, and lifestyle habits
  • Opportunities to participate in program-based initiatives and supportive activities

This approach recognises that sustainable weight management is best achieved through consistent support, education, and realistic lifestyle changes, alongside medical care where appropriate.

Other Services Currently Available

While general primary care and GP services are paused, St Clair Medical Practice continues to provide:

  • Follow-up of test results ordered through the practice
  • Immunisations and vaccinations
  • Iron infusions
  • Medical record access and transfers

Patients requiring ongoing general medical care are encouraged to arrange this with another medical practice during this period.

Looking Ahead

We look forward to sharing additional service offerings and enhancements as part of this specialised program. Further updates will be announced by St Clair Medical Practice.

We are excited about the care we can provide through this focused service and the opportunities it creates for patients.
